Welcome to Evans Dermatology!  We are gladly accepting new patients at this time.

 

We would like to take this opportunity to welcome you to our office!   This page contains answers to some of your most commonly asked questions by patients entering our practice.  We hope you find this information helpful. We accept most insurance plans, including but not limited to, Medicare, Tricare, Blue Cross Blue Shield, Humana, United Healthcare, Cigna, and Aetna. Most insurance plans do not require a referral to schedule a dermatology appointment. We can not determine if your insurance requires a referral until we actually see your insurance card.   Please check with your insurance plan to verify if you need a referral to be seen. If you come to your appointment and we then see that your insurance requires a referral, you will not be able to be seen until a referral is obtained!  We do not accept any type of Medicaid plan. Tricare Prime requires a referral. We are happy to see patients of all ages, ranging from pediatric to geriatric.

New Patients

Your First Visit

Please arrive 10-15 minutes ahead of your scheduled appointment time to fill out new patient paperwork. This may also be done prior to your appointment through the link in your email. Please bring your insurance card and photo identification with you to your appointment. Minors under 18 years old will need a parent or guardian present. 

 

Being Well Prepared

 

Being well prepared for your first visit will ensure that the doctors have all of the needed information to provide the best possible care for you.   It will also help to alleviate and unnecessary anxiety you may be feeling prior to your first appointment. We invite you to take a moment and familiarize yourself with the providers HERE

Please arrive without makeup on. Masks are optional in our office, and if you would prefer your provider to wear a mask, please let us know.

Ask one of our friendly patient service representatives about our patient portal.   Activation of our patient portal is required for use.  Through our patient portal you can answer our health questionnaire, request appointments, request prescription refills, and communicate with our clinical staff.  For quicker response on message, please us the patient portal.  Please allow up to 48 hours for phone messages to be returned.

Once your portal is activated, you will receive an email with instructions on registering for our patient portal.  Email link expires in 72hrs, if link expires click forgot password to reset login.  Please fill out all sections under "My Health", this will make your check in process quicker.
